This happens probably because you don't have rlm_sql_mysql compiled. How did you install mysql? from source or rpm...I had the same problem, because first I had mysql from rpm...and I didn't have mysql.h...and some other header files...so I installed mysql from source...and after then I reinstalled freeradius and after that I had rlm_sql_mysql.
