Those are just your regular bad logins, nothing to worry about, just
check if both the user and the radius server are in agreement on what
the password should be.

I get/got these for a bit just after switching the NAS router from the
piece of excretia pretending to be a radius server on the old winNT box
at work to the new freeradius server on the linux box. Essentially, the
login record was sent to the old machine since it was still talking to
that one at the time, then the switch occurred, and when the user logged
out the logout record was sent to the new server, generating this error.

Shouldn't happen again after a while.
