  No, you didn't.  If you had, it would have worked.

  In fact, if you just configured a clear-text password for the user,
EAP-MD% would work out of the box, without any changes to the
configuration files.

  Read the rest of the debug output to see what Auth-Type you set, and
why it isn't working.

  Of course not.  EAP doesn't send passwords.
