I think I've found a possible bug in rlm_exec (???).
Something like:
exec myscript {
...
program = "/path/to/myscript.sh %{Packet-Type}
...
}...results in a correct first argument myscript.sh for Access-Request and Accounting-Request, but not Access-Accept. An Access-Accept packet causes the script to be invoked as though the packet were an Access-Request.
