Yes. The NAS is broken.

  It *may* be possible to work around it a little, with hacks to the
server.  But I'm not sure I'd recommend that.

That's what I figured too.

Could I request that FreeRADIUS logs an error message when this condition occurs (ie. a reply is generated on basis of src IP/port & authenticator)? The lack of any logging information at all (even at -X level) made this very difficult to trace...
