for me Max-Daily-Session and Max-Monthly-Session acts as simple 
Session-Timeout (someone has got them to work as they are made for?), 
but Max-All-Session works as it should.

I don't understand WHERE is that password missing: any ideas?
   

 You're asking the server to authenticate someone, but not telling
the server how.
