And right before that in the log it shows you WHAT script it's running, and WHY it failed. If you want to solve the problem, don't delete every piece of useful information from the logs you post to the list. The debug output shows you the ntlm_auth command that the server is running. Since it works when you run it from the command line, the obvious next step is to _compare_ the two. Then, if there are differences, make the BROKEN one more like the WORKING one.
