I decided to take your advice and create a CD with just Xorg and XFCE4 on it. The result was the same. This time I did notice that /usr/home/freesbie/.freesbie_tmp/X11R6/bin/ contains startx and startxfce4 (even after I run scripts 7, 8, and 81) so those programs appear to be added using 6.packages.sh they're just not being put onto the CD.

Again, I'm planning on rebuilding my system from scratch when FreeBSD 5.4 becomes official unless this helps someone see something that's causing my system to not build the ISO properly.

Try to isolate the problem choosing only few packages discovering
the one(s) with problems.
