Log message:
Reversed the note_on and note_off commands. This should result in a
very usable adlib driver. As soon as I dig up the relevant docs, pitch
bend should be in the adlib and mt32gm drivers too.
The voice allocation code can probably be completely ripped out because
it was written with the wrong set of assumptions.
Please test, as I have no FM hardware to test this with. :)
