1) Pass along the AllStop CC (0x7b)
2) AdlibEmu driver now handles the AllStop CC.
3) Fix bug #341 (crash on running out of voices)
(3) was embarassing. (For the record, 0x19 +1 != 0x20). This bug was
only triggered if you were using the 12-voice adlib hack. That
screwed up offset was causing memory corruption, and occasionally
crashes.
Anyway, test away. I'm going to get breakfast now.
