I sincerely apologize for misleading you. I forgot that Leon's new
StartForm functions takes care of all of this automatically.

No longer necessary (based on StartForm).

I don't know. However, I just did a cvs update, dumped my entire
database, re-built the database, and I was able to login okay. So, at
least the version in CVS is working correctly.
