>From dmesg I find the basic problem is that tuner is not loaded correctly.

When you have a PVR-350 with this tuner add the below to /etc/modprobe.conf 
option ivtv tuner=38

This should not be needed on kernel version >=2.6.19

I am running 2.6.15.-22-386 on a debian system
when i add option ivtv tuner=38 to /etc/modprobe.d/modprobe.conf it complains 
about a bad line.
If i make a file /etc/modprobe.d/ivtv and put that line in it still complains 
about a bad line.
Does any Debian user know the answer?
