Just some general suggestions, my apologies if some have already been
suggested:
1. Differences in behavior from Netscrape and IE?
2. try passing the fields without a secure server prefix (http instead of
https). I have had this problem before where the info from form2 was sent
but not from form1 because of a glitch with the secure server.
3. rename every field of the form to rule out a reserved word conflict.
4. try the same application on another machine with CF401 installed instead
of CF451 and/or and older service pack
Sincerely,
Shane Witbeck

We're using 4.5.1 and there are no upload forms anywhere on the site.

Which version of CF server are you using? CF 4.0 has a known bug that
mangles Form.Fieldnames when there are file upload elements on the form.
Could that be it?
