Hi Donald,
 
I just think of the wireframe as documenting the Pages and Paths of a proposed application.
 
The wireframe contains text descriptions of every Page in the application.  Some people expand this to include all States (ie including actions states as well as display states).
 
Then you define every Exit from each Page.  These are usually links and form submits, though you can expand this to include "events" in the general sense.
 
Each Exit will point to a Destination, ie another Page of the application.
 
That's about it.
 
Download it and have a play.  I can send you a real-world example is you like.  See also Bob Silverberg's presentation from last years FB Conference.

I am curious is there a formal document that describes everything that goes into a wireframe?  Like what is is for, how to use it and what the final wireframe is to represent.  If any information can be supplied please send it.  I am thinking about using them with a new project and I want all the information about wireframes before I finish the application.
