Hello!  I just joined the list, skimmed a few hundred archived messages on the web, and feel ready to jump in.
 
 
How about something like this:
 
    <io>
        <in>
            <exception type="BadUsername" when="user name is blank or contains illegal characters" />
        </in>
        <out>
            <exception type="UnauthorizedUser" when="user is not authorized" />
        </out>
    </io>
 
As Steve suggested, exceptions that are thrown are listed in the <out> section, and exceptions that are caught are listed in the <in> section.
