As far as i know he had sent a sample to you (though i think it was just the PDF output file and not the Indesign test template,if this is the case I will have him send that in as well)

If I were to place a bet, I would venture its caused by some known issues with a particular version of InDesign--if this is the case, we still may be able to patch things up, even if it happens to be a problem in a particular InDesign.
