Kim,

You've got the right idea, identifying the different possible approaches
and trying to determine the pros and cons of each.  Here's what I would
recommend, based on your ideas:

A) Probably the easiest and fastest method, as those 5 color fill graphics
will be easy to create, and be fairly small files.

B) Would work, but multiple-page templates and OnRecordStart rules create
larger template files, are more complicated to set up the rules, and might
take slightly longer to compose.  Plus this would be harder to maintain if
you make any future template changes, you'd have to edit all 5 layouts. 
Save this approach for layouts with more complex variations.

C) A good idea, but not currently supported by Printable.  You can't
change properties of a frame (border, fill color, etc.) via rules.  Would
be great if Printable would add this as an upgrade.

There shouldn't be any issue with placing a text frame and a graphic frame
in the same position, just make sure that you order the layers properly,
so the text comes out on top.  If that doesn't work, or you need to
simplify it, you could make your colored images contain the text as well,
and just make a rule for one graphic frame.  But you don't have to, if you
prefer to print the text from the data instead.

To create the rule, I would recommend the built-in Switch Wizard, as
you're trying to choose 1 of 5 images based on a single value (title) in
your data.  The tutorials and documentation that come with FusionPro
(especially the Rules Guide) are pretty good for learning, combined with
the different rules templates and wizards that they've built in.
