I think we've discussed here before that part of the problem has been that at least in some African countries husbands work far away from their families to earn a living and used the services of prostitutes, so that when they returned to their wives, they infected mothers. So economics, not just human nature, has been a factor.
As with other areas in the Third World empowering women, there has been some success encouraging wives to insist on condoms.
