I thought just the recombinant BGH (rBGH, trade-name Prosilac) was  
banned in Europe in the mid-late 1990s. The "natural" form has been  
synthesized and used since the 1980s.... am I misinformed on this topic?

The use of rBGH is one reason that most nations will not import US  
beef & milk - and with good cause!

Another possibility is inequality in wealth distribution - as wealth  
is more equally distributed across a society, it seems that their  
members get taller. At least, that's the current 'most rational'  
explanation for the Dutch height issue (Dutch men average 6'1" now - a  
7 inch gain over the past 150 years or so).
