Is the script executable? Does it work when executed manually? Also
outside your user-environment?

The icons directory is icons/{22x22,32x32,48x48}/apps/

Didn't the app show up in the menu at all? Or just not with the correct
icon?
You're working in ~/.fvwm-crystal, right? Do other settings or changes
from that directory come into effect?
Are you subscribed to the list? Cc'ing bugs (me).
