Go into Keychain Access and delete the login keychain...the problem is  
that when you change the password via the install disk (or the command  
line passwd command), the keychain password isn't changed, so it's  
still set to the original, unknown password.

You lose anything stored in that keychain, but the next time you need  
to use it, OS X re-creates the login keychain with the current login  
password.
