Boot from the OS X install disk with Apple Hardware Test (AHT) on it and run the Quick and Extended tests. Chances are, nothing will pop up in the way of errors. Batteries die over time, and even though it's been plugged in that doesn't mean the chemicals inside the battery haven't been decaying. They have. Buy a new battery.
