Here is the process that I go through to copy a DVD.
Play DVD for a few seconds to unlock.
Use DVD Backup v1.3 to uncode DVD and put on hard drive
Use DVD2One X to remove the subtitles and extras
Use Toast Titanium to burn onto new DVD

*All File names must be in caps
*ON the final copied DVD there must be 2 files for most stand alone DVD players to 
read it,  VIDEO_TS  and   AUDIO_TS

I have found this to work the best after trying countless other ways.
