An OS 9.1 CD simply won't work.

The iBook 500 requires at least Mac OS 9.2.1.

I think that's an old page with the max OS listed, not the original. I have a 600mhz iBook (third rev, 16VRAM) that came with 9.1 originally.
For this particular problem it sounds like the OP probably didn't install OS9 drivers after reformatting the HD to install OSX. Might take another reformat to get it going.
