I peeled the IMS sticker off of the main (I assume) chip on the card and
after scraping off the VOID (no biggie I guess since they're out of
business) goo, I found the following:

PLX Technologies Inc.
1995
F6X0 129
PCI9060ES REV1

There is also another same size chip next to it that says:

NEC Japan
D98401GD
961RK003

Guess I should I try both of these vendor?
