Not really sure about the Model Number...on the card it just says ExpressPCI UL2D... The bottom part that goes into the PCI slot is bigger than the actual PCI slot (meaning there's more of it with little gold liney things) but there are two gaps so it fits in. Does that help? (probably not)
The previous owner (a well known G-List member who I trust) said it's a retail card and he had it in his beige G3 266 running with OS 9.1 for a while. The thing is I couldn't even get it to work while booting into 9.2.2 off of another drive. I'm not really sure about termination. I just unplugged the cable from the old ExpressPCI PSC Ultra Wide Card (ATTO) and plugged it into the new one. It should mean, since it was working perfectly and I'm booted off of it now, that the chain (just one drive and a multi connector type cable) is correctly terminated??
