It comes from Classic crashing. You know how OS 9.2 leaves those files 
in the trash after a crash (and even a proper shutdown a good deal of 
the time)? OS X's automatic fsck on restart never clears the resulting 
B-tree errors you've noticed. Running Disk First Aid (Disk Utility) 
after booting from the installer disk will, however. I believe (but 
won't swear to it) that multiple passes of fsck from single user mode 
will also properly clear these errors.

I tend to think this is a bug in the way OS X does autocheck on 
restrart. Perhaps there's some startup parameter that can be set that 
will correct it?
