HI,
        I would try another monitor, a multiscan one, to test your board.
Let me explain.
I suppose that your are using the Apple 14" monitor that replaced the 
original 13" in 1993.
The 13", and probably the 14" too, needs a sync signal where 
horizontal and vertical syncs are mixed.
The VGA interfaces delivers separated H-sync and V-sync signals.
I had this problem trying to connect my 13" to a Formac Proformance 40 board.
I solved the problem building a custom adapter that included one 
transistor and two resistors to make the mix.
