Used:
Adaptec Powerdomain 3940uw
Advansys 3940UW -now owned/supported by Initio
Domex 3940UW (rebranded from Advansys)
-Both the Advansys and Domex cards can be flashed to either Mac or PC,
and are quite reliable. I used one of each for over a year in my beige G3s.
If you can find them on ebay, you could probably get one for around $50-75.
You may need a PC to flash to the mac rom, I don't remember for sure though.
New (may be able to find used on ebay or elsewhere):
Adaptec Powerdomain 39160
Initio 9200U/UW
