What software or hardware is using sound? One possibility is that the 
sound hardware/driver is in use by something that doesn't release it on 
quit, similar to a 'cannot empty trash, file is in use' problem.

If that isn't helpful, sound settings are stored in PRAM. The forbidden 
resource offers these suggestions:

  -  Clear parameter RAM by holding down Command-Option-P-R during 
startup, but before the welcome screen
  -  (shutdown)
  -  Reset Cuda chip
  -  Reset logic board by removing the battery for at least 10 minutes.
