I have an MDD G4 dual 1GHz which I bought second hand.
Gradually the fan has become noisier and noisier.
I recall reading something about exchange fan, could have been someone selling a similar Mac and saying that they'd fitted new silent fan(s), but I'm not sure.
Apple provided a "Power Supply Exchange" program for $19.95 which gave you a new power supply, with quieter fans and a new main 120 mm fan to replace the original. It significantly reduced the noise of the "wind tunnel" MDD units. It was a time limited program. It's over now. Unless someone has one of the Apple kits available there's no way to get one from Apple.
Commercially there is an excellent solution available from Verax, although it is spendier, which offers dramatic noise reduction and improved cooling.
You can find lots of history and information about the whole MDD G4 noise issue at http://www.g4noise.com/
