I think the earliest B&Ws had 8.5.1 on them. By the time the rev.2 came out it they were all on 8.6 (my 350MHz rev.2 came with a MacOS 8.6 CD). MacOS 9 came out around the same time as the introduction of the G4, so I'm not sure if any B&Ws had it preinstalled.
