The first FireWire card I picked up for my B&W rev 2 was a USB 2.0/FireWire
400. It worked fine for 4 months. After 4 months it wouldn't recognize my
external FW devices until I powered them off and back on again. It got to
the point where the mac would not boot into OS X 10.3.4 on occasions. It
finnally stoped booting all together. I re-installed Mac OS X 10.3.4 but the
problem continued. Mac OS 9.2.2 booted fine. I picked up a cheap $30.00
Firewire card and a $20.00 USB 1.0 hub and have not had a problem since. I
don't have any USB 2.0 devices.

Just my experience.

Replace the USB only card with a combination USB-Firewire card. Several 
different brands that are known to play nice with Mac's.
