OK, so here's an update:
- I don't think RAM is the problem, because I have tried all 3 sticks individually and in various slots.
- I did a bad block check on the HD. Nothing came back.
- I zeroed all the bytes on the HD.
Now OS 9 installs successfully, but on reboot it does one of two things:
- it gets stuck at "happy Mac" - it shows the happy Mac and just hangs there, OR
- it bombs on bootup with a type 10 error.
I verified that the HD is indeed the secondary master. The CD-ROM is the primary master.
Disk First Aid has repeatedly shown filesystem errors, which I resolved either with Disk First Aid itself or by repartitioning or reformatting.
This OS 9 CD came with my 10.1 and is OS 9.2.1, but I don't think that makes a difference.
In the next week or so I should be able to swap out the HD, but I don't have any HD spares so I'll have to find another HD I can put into this system.
It seems to me all signs point to the HD, but I'm open to other ideas if you guys have any.
Thanks for the help, guys.
