Not necessary.  The Smurf works well with Jaguar, Panther, and Tiger.

The on-board IDE is Ultra ATA/33.

2) Is there a requirement that to be recognized boot CD drive has to
be at a certain location? (Master vs slave, "IDE" vs "Ultra ATA"?)

I have DVD-ROM and ZIP.  I couldn't get the DVD to work as a Slave.

3) I know that there are other standards and sub-standards relating to
the technologies and startup protocols that a CD drive will respond
to. How can I identify OS or hardware requirements? (I've seen one
Apple specification web page for this machine, and it's mute on this
level of detail.)

Apple's TIL and the service manual have details.

Apple doesn't support Cable Select, so don't even bother trying. Use standard Master-Alone or Master-with-Slave + Slave configs.

Also be aware of cable capacitance issues. Some cables just don't work if you turn them around.
