X11.app is, specifically, a server for the X Window System. The X Window System (which I shall call X11 for short) is a remote windowing system. It allows the execution of an application to be abstracted from the display of that system. Put simply, you can "run here and display over there."

X11 is the standard windowing system of most Unix systems - think Display PDF on OS X, and Display PostScript on NeXTSTEP. It has been ported to probably every modern OS, and with that server portability comes application portability: applications written for X11 can be compiled on any system with X11 libraries, and they can be displayed on any system capable of running an X11 server.

What X11.app gives you is that X11 server - the ability to display X11 apps on your OS X system. Those applications can run on your system, or they can be run remotely and displayed to your system.

The GIMP is an example of an X11 application. xterm, xeyes, xlogo, and all of the other x* programs are also X11 applications. (These are called X11 clients, in X Window System speak.)

As for where to get others besides The GIMP, the fink project probably has some, though I try to be an OS X purist and try to stay away from those. Don't ask me why, for I don't really have a good answer. ;)
