If you are running Tiger, try the KeyChain First Aid App.

In earlier versions of X (10.2 on, I'm sure) there's a repair keychain function in the Keychain Access utility.

Once I had this problem, though, and I had to delete the keychain 'login' entirely, and let OS X recreate it .
