One thing to try is after you reformat, erase using the write zeros
Option,
do it a few times, work it hard, then try copying a lot of real files.
The idea is try and make it break again if it will. "stress test" it.
Also do not forget to check SMART status in disk utility. If you have
administrative
savvy, you can download smartmontools source and build it which will
give you a complete report. However, I had a situation like yours
where SMART claimed the drive OK but it failed the stress test.
[Error logs found in Applications/Utilities/Console]
An error message like "i/o" error is obscure, but if you can check the
LBA (usually twice the actual bytes) you can sometimes format free
space around it. This worked for 4 years on my pismo, but on this
recent drive it only lasted for six months (before new defects ...)

Yeah I also suspected a machine problem, but swapping 2 machines /
drives it turned out the drive it was (well maybe some other factors
involved -- the machine was running hot because they blocked the
ventilation !!!)
