The first time you start the game, the game will not be outputting to
the clipboard, but will be running through nvda directly. And you
missed a step, it sounds like... you didn't actually switch
synthesizers and use the protalker English addon. If you do that, it
should be fine.

after you install everything you need and add the Japanese language.
