Yes, I've seen this bug as well. I believe this is related to the spikes 
bug where you can walk across them without taking any health damage or 
dying. What is likely happening here is you are landing on the spikes 
without dying, and then walking across them. Since there is not related 
sound  for walking over a spike it is defaulting to the last sound 
loaded which was the ledges.

Yeah, this happened to me as well. The reason is that the game isn't 
processing both actions simaltaniously so they are taken in the order 
the game processes the requests in the game loop.
I agree it is sometimes a really funny thing when you die, but you also 
get to kill the monster after you are dead. I can't tell you how many 
Atari games where the enemies got me, but as I am dying I also took them 
out as well. It was a real laugh.
There was this one Atari game, UFO, where you were suppose to fly over 
this planet zapping tanks and stuff with your lazers that was on the 
ground. I can't remember how many times I would get zapped by a tank and 
i would remain on screen a few seconds too long where I could blow up 
the tank or enemy that killed me before crashing into the ground. Either 
that or as I was crashing I would position the UFO over the enemy who 
killed me so i would crash into it killing it. Real funny way to go out 
in a bang.
