When you buy or find armor, like the jacket, it is equipped automatically. You will always be wearing the most powerful armor that you own. For example if you found a football helmet then that is what you would be wearing. If you then find a military helmet, you will be wearing that instead because it is stronger than the football helmet.
The game doesn't end no matter how many times you die. Perhaps you're just having some trouble navigating around in the safe zone. Try following the carpeting sound since carpet leads to the exit.
