evt1_MIGeoRef_imagine.img is the output from Erdas Imagine based on specifying 
EPSG 3078. This raster is in the correct location.
gdalinfo for these rasters is copied below. Am I missing anything with the gdalwarp usage?

I have not reviewed this in detail, but I would suggest filing a ticket on
the issue.  It isn't immediately obvious to me what is going wrong.
