Thanks for taking the time to check this. Given the care with which gdal is maintained and it's foundational position in the GIS world, I would have been surprised if it was not conformant to the standard.

I used the same utility you found and got the same results, which is what triggered the exchange with the developer of the PHP shapefile library. I will pass on this information.

David, what online utility have you used? I've uploaded driveway_points[.shp|.shx|.dbf] to https://www.litchiutilities.com/shapefileToCsv.php and while it generates 2 warning messages* it also nevertheless creates the requested CSV file.
