SOLVED... I had already discovered some time ago that offline files
are case-sensitive, so all references to files, in the HTML and the
manifest have to match exactly, including the case, otherwise they
can't be served locally. My application is fairly complex, and it
turns out I had missed one lowercase 'o' that should have been an 'O'.
I discovered it by browsing the files with Firebug while working
offline, and seeing that one of the scripts couldn't be displayed.

It would be good to make that case-sensitivity an option in future
versions though.
