For small pads, like TSSOPs, the best way is to draw a line connecting
the pad to the polygon.  Think of it as a one-armed thermal.

For large pads, like when building a switching power supply or
attaching a thermal pad, to make a solid connection to the polygon I
use four lines to "fill" the clearance gap.  These four lines form a
rectangle that just covers the gap.
