Yes, the rule should be that personally identifying images should be routinely suppressed in the absence of permission from an identifiable person who has signed an informed release. An informed release would include a declaration that they understand how open source image licensing works. There are privacy issues. Can a person remain anonymous and release an image of themselves. If so, how, in the case of Commons or Wikipedia? We routinely use the oversight tool on the English Wikipedia to suppress phone numbers and home addresses as releases of personal information.
