Hey Neil, the fact that you only see this sometimes make me
suspect that the problem may be browser caching.  Some
browsers get very clever about caching, even of posted
form data, and it may be that you are running afoul of
this.  One thing you can do is have your XQuery emit
a no-cache header to ensure the browser doesn't
try to outsmart you.

In MarkLogic you would do it like this:

xdmp:add-response-header("Pragma", "No-Cache")
