I would be overjoyed to get a hot-linked index. I'll give you the Microsoft Word source, or the HTML that it produces when I create a Web page, and you can go to it.
If I give you the source, you could add to the index callouts if you like. I found that to be a surprisingly onerous task. The web pages are built as follows: I save the source in Word as HTML; I run a script to find the chapters & split them into pages; the script installs the navigation header/footer in each page. The Index chapter is deleted because it is meaningless. If this seems OK, email me off-list & we'll get started.
