Hello,
There are several tracks in the UCSC Browser that may be useful for 
you.  Each track has a track description page that describes the methods 
and sources. If none meet your exact needs, the Table browser can be 
used to link (by genomic overlap) data between two tracks of interest 
(for example SNP 129 and GAD View).

For human, start with the track groups "Phenotype and Disease 
Associations" and "Variation and Repeats" and "Pilot ENCODE Comparative 
Genomics and Variation". The track UCSC Genes from the group "Genes and 
Gene Prediction Tracks" also contains some of this type of information 
on a per-gene basis.
