Hi Sung,

There are two ways to use liftOver.  You can use the web-based, version, 
here: http://genome.ucsc.edu/cgi-bin/hgLiftOver by pasting or uploading 
your coordinates.  Or you can use the "liftOver" command from the Genome 
Browser source tree.

Are you using the command-line version but just looking for the 
hg18ToHg19.over.chain file?  If so, you can find it here:

http://hgdownload.cse.ucsc.edu/goldenPath/hg18/liftOver/

(All of the over.chain files are stored in the directory corresponding 
to the "from" assembly.  So, all of the files that convert from hg18 to 
something else are in this directory.)
